From forward funnel it appears to be the Doc logo so it's likely to be the Piedmont. The aft guns have been removed and a helicopter pad and a hanger have been added. The forward guns and fire control director are still present.

This is a model of the Piedmont or of any Dixie-class tender as there are no markings. Who created the model and its construction was not sent with the photo.
